Annex West Health Center in West Philadelphia has been offering care to patients and the community since 2015, in partnership with the medical case management team at Action Wellness. As part of the Family Practice and Counseling Network (FPCN), Annex West provides services regardless of insurance status or ability to pay. Annex West delivers comprehensive, patient-centered, culturally competent and high-quality primary health care to some of the Philadelphia area’s most vulnerable individuals and families. Annex West services include chronic disease management; physical exams and preventive health care; family planning; STD testing; substance abuse disorder treatment; Hepatitis C treatment; HIV testing, prevention and care. Our behavioral health staff assists patients in reaching their health goals and in finding the ongoing mental health care and support they need. Annex West upholds the FPCN value that healthcare is a right and not a privilege.
