PHMC Care Clinic

The PHMC Care Clinic is a Federally Qualified Health Center that provides quality community-based primary care to adults requiring specialized health care, including HIV/AIDS, Hepatitis C treatment, and family planning. PHMC Care Clinic also provides evaluation of mental health concerns, patient education courses and connection to health care specialists. Substance abuse medication (Suboxone, Vivitrol) is also available in addition to peer support and case management for specific populations. The PHMC Care Clinic has a friendly bilingual staff and interpretation services available. PHMC Care Clinic is a Patient-Centered Medical Home.
