Rexdale Community Health Centre (Rexdale CHC Main Office)

Rexdale Community Health Centre delivers integrated, coordinated clinical services through a team of physicians, nurse practitioners, nurses, dietitians, social workers, a physiotherapist and a chiropodist. Our multi-disciplinary team addresses the health needs of individuals and families at all ages and stages of their lives. In collaboration with the City of Toronto’s Public Health department, Rexdale CHC also offers dental services for children, students and seniors.
